El Director de Servicios Sociales es un miembro clave del Equipo de Liderazgo de Ridgeview que planifica, organiza, desarrolla y dirige la operación general del departamento de Servicios Sociales para garantizar que todas las necesidades emocionales y sociales relacionadas con la medicina de los pacientes se cumplan de acuerdo con todas las leyes, regulaciones y estándares de Ridgeview aplicables. Facilitar todas las descargas de los residentes (ordenar la salud en el hogar, DME, ayudar con la transferencia a otras instalaciones) Llevar a cabo actividades de compromiso individual y grupal para los residentes, así como asistir con eventos comunitarios especiales. Planificar y coordinar un calendario mensual de actividades y eventos que proporcionen una variedad de experiencias centradas en los residentes y la familia. Participar en la planificación de descargas, desarrollo y ejecución de planes de atención a los residentes y evaluaciones de actividades.   Long-Term Care Social Services Director Opportunity! $1,000 Sign on Bonus! Ridgeview Health and Rehab Center is looking for a  Social Services Director  to join our growing team! The Social Services Director is a key member of the Ridgeview Leadership Team who plans, organizes, develops, and directs the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Ridgeview standards. The Social Services Director is also responsible for  leading the facilities' activity programs, which include developing and implementing programs geared toward various individual abilities and interests.   Your Job: Manage and direct the Social Services Department, and identify and provide for each residents social, emotional, and psychological needs. Understand the MDS process with ability to interview, input, and complete social service assessments. Accurately assess, document and provide interventions to meet psychosocial needs of all residents. Develop care plans based on assessments Maintain a quality working relationship with the medical profession and other health related facilities and organizations. Manage department to assure all psychological and psychosocial needs of residents are met. Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required. Provide effective facilitation of resident care conferences: schedule, send letters to families/ responsible parties, and participate in care conference. Facilitate all resident discharges (arrange for home health, DME, assist with transfer to other facilities) Conduct individual and group engagement activities for residents as well as assist with special community events. Plan and coordinate a monthly calendar of activities and events that provide a variety of resident and family-centered experiences. Participates in discharge planning, development, and implementation of activity care plans and resident assessments Meets with administration, medical, and nursing staff, as well as other related departments to coordinate and plan activity programs Involves the resident/family in planning objectives and goals for the resident care planning Performs administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Executive Director as required Checks supply rooms to ensure that needed activity supplies are on hand to conduct scheduled activities.
